Public Records Request
Are you looking for information? If so, click here for our Public Records Request form, or download and fill out the pdf below. Once completed, bring the form and a 10% down payment to the Lewis County Transit office at 212 E Locust Street, Centralia. We are open from 8:00 a.m. to 4:00 p.m. Monday through Friday.
Public Records Request Cost:
- $0.15 per page for photocopies for any public records request
- $0.10 per page that is scanned into an electronic format
- $0.05 per each four electronic files or attachments uploaded to E-mail
- $0.10 per gigabyte for the transmission of public records in an electronic format
- The actual costs of mailing and mailing supplies of any public records request
A 10% down payment of the estimated cost will be required in order to start processing the request.
